All you have to do is fill out this online form or call 607-737-4535 TWO OR THREE days before the scheduled visit.

All you need is the following information:

• Insurance information
• Employer information
• Your doctor’s name
• Tests, diagnosis and/or codes for which you have been referred

YOU MUST STILL BRING PAPERWORK FROM YOUR PHYSICIAN OFFICE TO HOSPITAL ON DAY OF APPOINTMENT.
